Winter Conference Awards Application

SCHCA holds an award luncheon each year at our Winter Conference Meeting, on November 30th at the Hilton Downton Columbia, to recognize an outstanding Certified Nursing Assistant, LTC Nurse, Volunteer, and John P. Barber Service award of the year. Below is the selection criteria for each award and the requirements each nominee must meet to be considered. Please take a moment and fill out the form below to recognize an exceptional employee at your facility!

 

November 15th is the deadline for submissions.

 

Nominee Requirements:

  • The nominee must have a minimum of one year experience in a long term care facility
  • The nominee must be currently employed in a SCHCA member facility
    • at the time of application, and
    • at the time theĀ  award is presented

Selection Criteria

  1. John P. Barber Service Award
    • The goal of this award is to recognize an outstanding LTC Administrator, emphasis will be placed on recognizing an individual who:
      • is compassionate, caring and innovative in dealing with residents, families, and staff
      • has shown a dedication to continued commitment and development; and
      • has an active participation in SCHCA activities/efforts
  2. Volunteer Award
    • Volunteer Nominee’s will be ranked in each category based on the following criteria:
      • Leadership role in initiating programs
      • Ability to help residents realize their potential
      • Overall impact on residents
      • Involvement in activities
  3. Certified Nursing Assistant Award
    • The goal of this award is to recognize outstanding Certified Nursing Assistants, emphasis will be placed on recognizing individuals who:
      • are compassionate, caring and innovative in dealing with residents, families, staff and in the delivery of nursing care in the long term care setting; and
      • have shown a dedication to continued commitment and development
  4. Long Term Care Nurse Award
    • The goal of this award is to recognize outstanding LTC Nurses, emphasis will be placed on recognizing individuals who:
      • are compassionate, caring and innovative in dealing with residents, families, staff and in the delivery of nursing care in the long term care setting; and
      • have shown a dedication to continued commitment and development
