Course Description: This presentation is designed to strengthen clinical decision-making and interdisciplinary accountability in fall risk identification and prevention. The information goes beyond routine screening tools to emphasize critical thinking, timely reassessment, and resident-specific risk analysis. Participants will explore how clinical changes, medication, environment, and systems of care interact to increase fall risk, and how gaps between assessment, care planning, and implementing contribute to adverse outcomes.
