Course Description: Improving Quality Measure ratings has become a key focus for SNF providers, with an emphasis on enhancing the quality of care. Often, the efforts are reactive with a focus on removing the “trigger” from QM reporting or implementing temporary solutions that are not sustainable. This presentation offer practical strategies using a QAP-driven approach to uphold lofty standards of quality care consistently. Participants will learn data-driven methods to set quality improvement goals, develop effective interventions to enhance quality of care, and achieve sustainable outcomes.
